Electronic health records (EHR) have become standard in doctors’ offices and health care organizations to document patient visits, symptoms and conditions, test results and treatment regimes. The broad implementation of EHR is the result of the Health Information Technology for Economic and Clinical Health (HITECH) Act, which was intended to motivate the implementation of EHR and supporting technology. Its goal was to reduce the cost of health care by facilitating the exchange of electronic protected health information among doctors, hospitals and other health care entities.
